PATIENT WAS APPARENTLY ASYMPTOMATIC 20 DAYS BACK THEN DEVELOPED SWELLING OF BOTH LOWER LIMBS INSIDIOUS IN ONSET , GRADUALLY PROGRESSIVE , PITTING TYPE

C/O FACIAL PUFFINESS SINCE 20 DAYS

C/O SOB SINCE 4 DAYS INSIDIOUS IN ONSET GRADE 4 NYHA GRADUALLY PROGRESSIVE

H/O ORTHOPNEA

H/O CHEST PAIN , FEVER, BURNING MICTURATION, ABDOMINAL DISTENSION

NO H/O ABDOMINAL PAIN

NO H/O PALPITATIONS, DECREASED URINE OUTPUT
