I have been given this case to solve in an attempt to understand the topic of " patient clinical data analysis" to develop my competency in reading and comprehending clinical data including history, clinical findings, investigations and come up with diagnosis and treatment plan

PATIENT WAS BROUGHT TO CASUALITY WITH COMPLAINTS OF BREATHLESSNESS SINCE 4 DAYS

C/O SWELLING OF BOTH LOWER LIMBS SINCE 4 DAYS.

HOPI: PT WAS APPARENTLY ASYMPTOMATIC 4 DAYS BACK THEN HE DEVELOPED BREATHLESSNESS GRADE III NYHA , INSIDIOUS ONSET ,GRADUALLY PROGRESSIVE, AGGREVATED WITH WALKING AND RELEIVING WIT REST.

H/O ORTHOPNEA PRESENT.

C/O PEDAL EDEMA SINCE 4 DAYS , INSIDIOUS ONSET , PITTING TYPE

NO H/O PND, CHEST PAIN, PALPITATION, NO H/O FEVER, COLD, COUGH.

NO H/O BURNING MICTURITION, DECREASED URINE OUTPUT.

PAST H/O :

K/C/O PULMONARY TB 1 YEAR BACK AND USED ATT.

NOT AA K/C/O HTN, DM, CVA, CAD, ASTHMA, THYROID DISORDERS.
